Irritable bowel syndrome (IBS)


Irritable bowel syndrome (IBS) is a group of persistent or intermittent attacks, with abdominal pain, bloating, bowel habits and (or) changes in stool characteristics as clinical manifestations, and lack of gastrointestinal structure and biochemical abnormalities in the intestine Dysfunctional diseases.

The etiology and pathogenesis of IBS are not yet fully understood. It is considered to be the result of a combination of factors such as abnormal gastrointestinal motility, abnormal visceral sensation, abnormal brain and bowel regulation, inflammation, and psychology.

IBS has no specific symptoms but compared to organic gastrointestinal diseases, it has some characteristics: slow onset and intermittent attacks; long course of the disease but the general health is not affected; the appearance or aggravation of symptoms is often related to mental factors or stress Status is related; obvious during the day and relieved after sleep at night.

 Symptoms

(1) Abdominal pain or abdominal discomfort is the main symptom of IBS, accompanied by abnormal stool frequency or shape. Abdominal pain is more than relieved after defecation. Some patients tend to appear after eating. Abdominal pain can occur in any part of the abdomen, localized or diffuse. , The nature of pain is diverse. Abdominal pain does not get worse, and there are very few people who wake up with pain after sleep at night.

(2) Diarrhea Persistent or intermittent diarrhea, low stool volume, mushy, with a lot of mucus; The symptoms disappear after 72 hours of fasting; It does not appear at night, which is different from organic diseases; Some patients can be Induced by eating; Patients may have alternating diarrhea and constipation.

(3) Constipation Difficulty in defecation, dry stool, less volume, more mucus, constipation can be intermittent or alternate with diarrhea, often accompanied by incomplete defecation.

(4) Abdominal distension is heavier during the day, especially in the afternoon, and relieved after sleep at night.

(5) Upper gastrointestinal symptoms Nearly half of patients have upper gastrointestinal symptoms such as heartburn, nausea, and vomiting.

(6) Extraintestinal symptoms: Back pain, headache, palpitations, frequent urination, urgency, sexual dysfunction, and other extraintestinal manifestations are significantly more common than organic bowel disease. Some patients still have manifestations of mental and mental disorders of varying degrees, such as anxiety, depression, nervousness, etc.

Signs

Usually no positive findings, or only mild abdominal tenderness. Some patients have symptoms of autonomic nervous system disorders such as hyperhidrosis, fast pulse, and high blood pressure. Sometimes the sigmoid colon or painful intestinal loops can be touched in the abdomen. Digital rectal examination may feel anal spasms, high tension, and tenderness.

Chinese medicine treatment

Chinese medicine divides IBS into large intestine dry-heat syndrome, cold-heat mixed syndrome, spleen and stomach weakness syndrome, liver-qi stagnation and spleen syndrome, liver stagnation-qi stagnation syndrome and other syndrome types.
